Residents
A RESIDENT can set up an account except for an ENDO resident. After setting up an account through the web or manually, please email or fax to us proof of residency, such as your ID. We will need this information to approve your account and so we can provide you with resident discount prices.

Faculty
After setting up your account online, please specify if you are PART-TIME or FULL TIME faculty and email or fax to us proof such as ID.
